 Notes: For operators of communication equipment sending overseas, like broadcasting or for operator of cockpits, cabarets, night or day clubs, boxing exhibitions, professional basketball games, jai-alai and race tracks.

  • To submit the form 2551Q, you must:
    1. Sign in black ink.
    2. Get all copies stamped upon payment.
    3. Take one stamped copy back for your own records.

  • The form 2551Q is to be submitted on the january 20th, April 20th, July 20th, October 20th (20 days after the end of the quarter — If this day is a Saturday, Sunday or holiday, it must be submitted by the first working day AFTER.)
